Alternative Family Living (AFL) Provider host provides individuals classified with an intellectual/developmental disability or traumatic brain injury an opportunity to progress toward their highest potential in a least restrictive home environment appropriate to their need. A Host Home is a private home; a natural setting where the services and/or habilitation are rendered to the individual while living in a safe and caring family atmosphere. Duties It is expected that an AFL host will assist the person entrusted into their care with 24/7/365 oversight. This person actually becomes a member of the family and is treated as such. Some of the skills that the AFL host will assist person with are:
Personal grooming and cleanliness
Bed making and household chores
Eating and the preparation of food, and
Adherence to all medical directives and medications
Participation in community events and therapeutic activities
The social and adaptive skills necessary to enable the individual to reside in the least restrictive and most normalized community-based residential setting possible (i.e. non-institutional setting)
The AFL host will be equipped with the knowledge and resources to be able to assist the individual in achieving success, improving their sense of self by achieving goals, including daily living skills and appropriate socialization within the family and in the larger community AFL Host SkillsPreferred
Strong interpersonal skills with the ability to communicate effectively with physician, guardian, client & therapeutic providers.
Experience in caregiving or home care is a plus, particularly in working with individuals with disabilities.
Knowledge of medication administration and vital signs is beneficial but not required.
Proficiency in meal preparation and maintaining cleanliness, as well as teaching in these areas.
Ability to keep records and provide documentation regarding their client's progression towards goals, behavior etc.
AFL Host Requirements
AFL host must have a high school diploma or GED
Upon approval, AFL host must successfully complete First Aid, CPR & DMH/DD/SAS Core Competencies and required refresher trainings.
AFL host shall have a criminal record check performed, as well as a healthcare registry check in accordance with 10A NCAC 27G.0200 for staff.
AFL host shall maintain a current annual physical/TB test.
All AFL hosts (paraprofessionals) providing direct care shall be supervised by a Qualified Professional. Supervision will be provided according to supervision requirements according to licensure or certification requirements of the appropriate discipline.
If providing transportation, AFL host must have valid NC driver's license and a safe driving records, have an acceptable level of automobile liability insurance and vehicle must have passed a NC Vehicle Inspections/Emissions within the year.
Provide a clean, comfortable, client concise dwelling while serving as an AFL host.
Have a willingness to learn about different disorders and conditions that our demographic served may suffer from.

Duties
Maintain the health and safety of client at all times.
Complete all necessary documentation a timely and accurate manner.
Provide services to individuals in the home and/or community setting.
Provide training on self-help skills to enable a person with meeting basic life needs such as food, hygiene, appearance, and health.
Provide training in independent living skills and activities that enable a person to live a full life in the home and community (meal preparation, home management, community resource utilization, and self-administration of medication).
Provide training in mobility and activities that enable a person to move from one place to another in the home and community (gross motor skills, fine motor skills, transfers, access/use of public transportation etc).
Provide training in socialization and activities that enable a person to acquire new behaviors, increase fluency of skills, promote generalization of skills, and prevent regression of skill development.
Promote positive interactions with client, family, qualified professionals, all staff of EHS, and all team members involved in treatment.
Maintain confidentiality of client information by following applicable privacy laws.
Communicate regularly either verbally or written with EHS personnel, qualified professionals, and clients as necessary.
Qualifications:
High School Diploma or GED and at least one year of experience in a similar role.
Lives in or within commuting distance of Orange, Durham, Wake or Mecklenburg county.
Ability to submit to a background check and NC Nurse Aide Registry check in accordance with local rules and regulations.
Experience working with individuals with intellectual and developmental disabilities and/or TBI preferred.
Valid Driver’s License and automotive insurance.
A flexible schedule in order to meet the participant’s desired hours for services.
